APPLICANT REQUESTS THAT: 

 

The Fitness Assessment (FA) dated 13 Mar 12 be removed from the 
Air Force Fitness Management System (AFFMS).

APPLICANT CONTENDS THAT: 

 

He was injured during the FA and was not able to complete the 
sit-up component. He did not realize the extent of this injury 
until he finished the FA and reported to the flight surgeon. 

 

In support of his appeal, the applicant provides a copy of the Fitness Assessment (FA) Member Illness/Injury Claim Form and 
various other documents.

STATEMENT OF FACTS: 

 

On 20 Aug 12, AFPC/DPSIM sent a memorandum to applicant 
requesting additional documentation. Specifically, he needs to 
provide a copy of his AF Form 108, Physical Fitness Education 
and Intervention Processing; a signed score sheet and 
questionnaire for the FA in question.

AIR FORCE EVALUATION: 

 

AFPC/DPSIM recommends denial, stating, in part, that the 
applicant did not provide the requested documents. In addition, 
the FA Member Illness/Injury Claim Form submitted by the 
applicant did not reflect his commander’s approval/disapproval.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

A copy of the Air Force evaluation was forwarded to the 
applicant on 6 Nov 12 for review and comment within 30 days. As 


of this date, no response has been received by this office 
(Exhibit C).

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by 
existing law or regulations. 

 

2. The application was timely filed. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice warranting 
relief. After thoroughly reviewing the evidence of record and 
noting the applicant's contentions, we believe that relief is 
warranted. While we note DPSIM recommends denial, given the 
apparent support from his commander and the medical provider 
acknowledging that he had a legitimate injury, we find the 
applicant has presented substantial evidence to support the 
removal of his FA dated 13 Mar 12. Accordingly, we recommend 
that his record be corrected as indicated below.

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air 
Force relating to APPLICANT, be corrected to show that the 
Fitness Assessment, dated 13 Mar 12, be removed from the Air 
Force Fitness Management System.
